I have a limited number of slots for reduced rate, VOC, and select insurances. Eligibility decisions for one of these slots are made on a case by case basis and depend on several factors. If there are no spaces available, you can request to be placed on the wait list, and you will be contacted when your name gets to the top of the list on a first-come first-serve basis.
